Waiver Membership Information, Liability Waiver and Media Release Form

In consideration of being permitted to participate in any way in Ambition Athlete Development training programs, recovery services, or related activities collectively referred to as “Ambition Activities,” I, for myself or my minor child, and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby acknowledge and agree to the following:

Assumption of the Risk

I understand that participation in Ambition Activities involves inherent risks, including but not limited to:

• Minor injuries such as scratches, bruises, and sprains

• Major injuries such as joint injuries, concussions, staph infections, and heart related incidents

• Catastrophic injuries including paralysis or death

Medical Expenses Responsibility

I understand and agree that I am solely responsible for any medical costs, expenses, or insurance coverage resulting from injury or illness sustained during participation in Ambition Activities. Ambition Athlete Development will not be held responsible for any related medical bills or financial obligations.

Waiver and Release of Liability

I release, waive, and discharge Ambition Athlete Development, its owners, staff, coaches, contractors, and affiliates from any and all liability for injuries, damages, or losses, including those caused by negligence, arising from participation in Ambition Activities.

Indemnification and Hold Harmless

I agree to indemnify and hold harmless Ambition Athlete Development from any claims, actions, suits, attorney fees, costs, or expenses arising from my or my child’s involvement in any Ambition Activities.

Media and Photography Release

I understand that group and individual photos or videos may be taken during training sessions for promotional or educational use.

Behavior Policy

Ambition Athlete Development reserves the right to suspend or discontinue participation for any individual whose behavior is unsafe, inappropriate, or disruptive to the training environment.

Severability

If any portion of this agreement is held to be invalid, the remainder shall remain in full force and effect under the laws of the State of California.

Acknowledgment of Understanding

Please review and ensure you agree to all of the following statements before signing:

• I understand and voluntarily accept all risks associated with participation in Ambition Activities.

• If the participant is a minor, I, as the legal guardian, accept full responsibility for their participation and confirm that I have the legal authority to sign on their behalf.

• I confirm that I or my child is physically and medically cleared by a licensed physician to participate in physical training. I understand it is my responsibility to consult a physician before engaging in strenuous activity.

• I have accurately disclosed all relevant medical conditions that may affect participation.

• I understand that group and individual media may be used for marketing or educational purposes.

• I give permission for emergency medical treatment in case of injury or illness.

• I agree to follow all behavior and safety expectations set by Ambition coaches and staff.

By placing my signature below, I affirm that I have read this agreement in full, understand its contents, acknowledge that I am giving up certain legal rights, and sign this freely and voluntarily.

Special Event Registration Policy

By registering for any Ambition Athlete Development camp, clinic, or special event, I understand and agree to the following:

Final Sale Policy

All camp, clinic, and special event registrations are final sale. Once payment is completed, there are no refunds, credits, transfers, or make-up sessions for missed days, cancellations, schedule conflicts, illness, injury, travel, weather-related changes, or any other reason.

Non-Transferable Registration

Camp registrations are non-transferable and may not be transferred to another athlete, sibling, family member, future camp, membership, private session, or other Ambition Athlete Development service.

Injury, Illness, or Medical Situations

I understand that injuries, illness, or unexpected medical situations may occur before or during camp. However, camp registration remains final sale and does not qualify for a refund, credit, transfer, or make-up session.

Missed Sessions

If my athlete misses any portion of camp for any reason, I understand that the missed time cannot be made up, credited, refunded, or transferred.

Camp Schedule and Changes

Ambition Athlete Development reserves the right to adjust camp schedules, session structure, coaches, location, or programming when necessary due to safety, weather, facility availability, or operational needs.

Athlete Conduct and Participation

Athletes are expected to follow coach instructions, respect other athletes, and participate safely. Ambition Athlete Development reserves the right to remove an athlete from camp due to unsafe behavior, disrespectful conduct, or failure to follow instructions. Removal from camp does not qualify for a refund or credit.

Parent/Guardian Agreement

By completing registration, I confirm that I have read, understand, and agree to this Camp Registration Policy.
